{"id":2899,"date":"2017-03-30T10:34:00","date_gmt":"2017-03-30T08:34:00","guid":{"rendered":"https:\/\/www.acotv.org\/correspondencia-anomala-cortical-teoria-y-clinica\/"},"modified":"2022-03-31T11:58:44","modified_gmt":"2022-03-31T09:58:44","slug":"correspondencia-anomala-cortical-teoria-y-clinica","status":"publish","type":"post","link":"https:\/\/www.acotv.org\/es\/correspondencia-anomala-cortical-teoria-y-clinica\/","title":{"rendered":"CORRESPONDENCIA AN\u00d3MALA CORTICAL; TEOR\u00cdA Y CL\u00cdNICA"},"content":{"rendered":"\n<p class=\"has-text-color has-medium-font-size wp-block-paragraph\" style=\"color:#008ad0\"><strong>Teor\u00eda&nbsp;<\/strong><\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\">La correspondencia cortical es el sistema que utiliza la visi\u00f3n para integrar las im\u00e1genes que provienen de los 2 ojos y dar informaci\u00f3n sobre la posici\u00f3n ocular respecto al espacio.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Hace tiempo se denominaba correspondencia retiniana, ya que anteriormente se cre\u00eda que era de origen retiniano. Para integrar la visi\u00f3n de los dos ojos debemos \u00abfocalizar\u00bb la m\u00e1xima visi\u00f3n en la f\u00f3vea, que son puntos correspondientes pero con diferente direcci\u00f3n visual. Cada elemento del campo visual ocular corresponde a un punto de proyecci\u00f3n cortical. As\u00ed, la correspondencia es a nivel cortical, espec\u00edficamente en el \u00e1rea visual primaria (V1). La correspondencia sensorial norma es cuando un objeto cae en puntos correspondientes. De esta manera si el cerebro interpreta un mismo objeto en la misma direcci\u00f3n conectada por la f\u00f3vea, le sirve de orientaci\u00f3n para saber d\u00f3nde est\u00e1n mirando los 2 ojos.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u00bfQu\u00e9 pasa si existe una desviaci\u00f3n ocular (estrabismo)? Las 2 f\u00f3veas no est\u00e1n alineadas, provocando que tengamos 2 im\u00e1genes completamente diferentes en puntos no correspondientes. Es ent\u00f3nces cuando aparecen las anomal\u00edas de confusi\u00f3n (2 im\u00e1genes superpuestas), diplop\u00eda (visi\u00f3n doble) etc. que puede encadenar una respuesta de supresion (desconectar la imagen de un ojo para evitar la confusi\u00f3n y \/ o diplopia).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A nivel cortical como respuesta beneficiosa se crea una adaptaci\u00f3n (una correspondencia sensorial) cambiando toda la estructura retiniana para que los 2 ojos fusionen e intenten crear estereopsis (visi\u00f3n en 3D). La direcci\u00f3n visual primaria del ojo desviado cambia a una localizaci\u00f3n no-foveal. Pero, puesto que la estructura retiniana con m\u00e1s detalle y precisi\u00f3n es la f\u00f3vea, el nuevo punto correspondiente no tendr\u00e1 la misma definici\u00f3n, con el que la nueva visi\u00f3n en 3-D siempre es de menor calidad, pero existente.<\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img fetchpriority=\"high\" decoding=\"async\" width=\"700\" height=\"478\" src=\"https:\/\/www.acotv.org\/wp-content\/uploads\/imatge2-2.png\" alt=\"\" class=\"wp-image-2889\" srcset=\"https:\/\/www.acotv.org\/wp-content\/uploads\/imatge2-2.png 700w, https:\/\/www.acotv.org\/wp-content\/uploads\/imatge2-2-300x205.png 300w\" sizes=\"(max-width: 700px) 100vw, 700px\" \/><figcaption><strong>Figura 1.<\/strong>La imagen de la izquierda muestra un sistema binocular que proyecta en puntos correspondientes (f\u00f3vea), creando la visi\u00f3n en 3D. Como respuesta de adaptaci\u00f3n el cerebro crea este nuevo punto como correspondiente con la otra f\u00f3vea, obteniendo una sensaci\u00f3n de profundidad. \u00abm<sub>ue\/ud<\/sub>= f\u00f3vea\u00bb<\/figcaption><\/figure>\n\n\n\n<p class=\"has-text-color wp-block-paragraph\" style=\"color:#008ad0\"><strong>Evaluaci\u00f3n cl\u00ednica<\/strong><strong>&nbsp;<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Tenemos varios m\u00e9todos cl\u00ednicos para evaluar la correspondencia an\u00f3mala (ordenados de menor a mayor disociando):<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Cristales estriados de Bagolini<\/li><li>Sinopt\u00f3foro<\/li><li>Filtro rojo<\/li><li>Test de Worth<\/li><li>Post-imagenes<\/li><\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">La evaluaci\u00f3n cl\u00ednica es importante para aplicar correctamente el diagn\u00f3stico y de manera m\u00e1s importante el tratamiento.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Explicaremos resumidamente y esquem\u00e1ticamente el Test de Bagolini<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">El Test de Bagolini utiliza unos cristales sin graduaci\u00f3n, estriados de manera muy fina. As\u00ed, las im\u00e1genes grandes como por ejemplo una cara, mesa, bol\u00edgrafo etc &#8230; no se distorsionar\u00e1n. La acomodaci\u00f3n no se ve afectada. En cambio, una luz puntual har\u00e1 que al pasar por el cristal se distorsione en direcci\u00f3n a la estr\u00eda, ya que act\u00faa como filtro laminar. De esta manera si proyectamos una luz y la observamos a trav\u00e9s de estos cristales veremos una l\u00ednea y en el centro la fuente de luz. Si tenemos 2 cristales en diferente direcci\u00f3n de estr\u00eda (normalmente 45 y 135\u00ba) dispuestos uno en cada ojo, visualizaremos una cruz oblicua en forma de X y la fuente de luz en el centro. El resto se ver\u00e1 relativamente bien, sin distorsi\u00f3n, por eso se dice que es un test poco disociador, ya que no altera mucho la realidad, haciendo el test m\u00e1s fiable.<\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img decoding=\"async\" width=\"677\" height=\"458\" src=\"https:\/\/www.acotv.org\/wp-content\/uploads\/imatge3-5.png\" alt=\"\" class=\"wp-image-2892\" srcset=\"https:\/\/www.acotv.org\/wp-content\/uploads\/imatge3-5.png 677w, https:\/\/www.acotv.org\/wp-content\/uploads\/imatge3-5-300x203.png 300w\" sizes=\"(max-width: 677px) 100vw, 677px\" \/><figcaption><strong>Figura 2.<\/strong>&nbsp;Funcionamento de los vidrios estriados. La luz que incide sale projectada en el mismo \u00e1ngulo que las estrias del vidrio, (\u03b1<sub>a<\/sub>=\u03b1<sub>b<\/sub>).<\/figcaption><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">La evaluaci\u00f3n se realiza con condiciones normales de iluminaci\u00f3n y la mejor prescripci\u00f3n de gafas, generalmente la luz puntual se sit\u00faa a 1 metro, pero se puede poner a cualquier distancia. No es v\u00e1lida la prueba si existe fijaci\u00f3n exc\u00e9ntrica, por lo tanto siempre se debe evaluar anteriormente. Siempre se debe realizar el cover test unilateral por realizado el test de Bagolini.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Algoritmo resumido del Test Bagolini:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Se presenta un algoritmo donde explica algunas de las respuestas m\u00e1s comunes que podemos obtener durante la prueba. Se han omitido las respuestas de supresi\u00f3n (la franja que no vea significar\u00e1 que el ojo suprime o no percibe la luz). Respuestas como correspondencias an\u00f3malas parad\u00f2giques tendr\u00e1n un patr\u00f3n similar al expuesto pero cambiando a base del prisma. Es decir, por ejemplo un caso de exotropia pero la respuesta en el test de Bagolini nos percibe diplopia hom\u00f3nima (los puntos se cruzan por arriba de la intersecci\u00f3n, indicando endotrop\u00edas sensorial).
